geodata-import-topo: Convert county and municipality codes to int16.
And add NOT NULL and UNIQUE constraints. This makes it possible to use
lm_topo250.lansyta(lanskod) and lm_topo250.kommunyta(kommunod) as
FOREIGN KEYs.
